Yapp Med

A guided consultation, with the patient's history at hand

Yapp Med keeps the whole consultation in one place: you walk through it step by step, see what you recorded at earlier visits next to each section, and print the medical letter and GDPR consent at the end.

The app builds the patient record from every consultation: vital signs, diagnoses, medication, history and allergies stay in the patient's file, each with its date and a link to the consultation that recorded it. Yapp Med is the clinical extension for Yapp Book and needs an active Yapp Book subscription.

What the doctor and the clinic get

Interactive consultation, with history at every step

Pick the patient and the consultation type, and the consultation opens as a step-by-step page built on your specialty's template. The step bar stays centred on the step you are working on.

  • Steps in the natural order of a consultation: reason, history taking, background, allergies, vital signs, examination, investigations, diagnoses, medication and recommendations.
  • Templates for general medicine, neurology, cardiology, dermatology, paediatrics, obstetrics and gynaecology, ENT, ophthalmology, endocrinology, psychiatry and dentistry.
  • A per-section history panel: for the current step you see what was recorded at earlier consultations, with the date, the consultation type and the doctor.
  • A trend chart for blood pressure, weight, pulse, glucose, oxygen saturation, temperature and BMI, shown once there are at least two readings.
  • An unsigned consultation can change type with "Convert": what you wrote is kept, and only untouched template text is replaced.

Medical letter and GDPR consent, printed at the end

The completed consultation generates the medical letter on your clinic's letterhead. Printing goes through the browser, so it works on any printer or as "Save as PDF".

  • A letterhead with the provider name, address, doctor, contact details and clinic logo, followed by the sections you filled in; empty sections are left out.
  • Space for the doctor's signature and stamp; once the consultation is signed, the letter stays exactly as it was issued.
  • If the patient's GDPR consent is not yet marked as signed, the GDPR form is ticked automatically so it prints in the same run as the letter.
  • The GDPR form can be the one built into the app or your clinic's own PDF, uploaded in settings; after printing you record whether the patient signed.

History is kept and visible at the next visit

The patient record grows visit by visit. At the next consultation the history appears right next to where you write, with no digging through the file.

  • Every step writes into the patient's file (vital signs, diagnoses, medication, history, allergies, treatments), and each row keeps its date and its consultation.
  • The patient's list of consultations stays in the record; open any earlier consultation in one click, including from the history panel.
  • View history for the current section only or across all consultations, and for the same consultation type only or for every type.
  • Each doctor sees their own patients, and changes to the record are written to the audit log.

The medical letter and, when needed, the GDPR form go to the printer or to PDF.

Frequently asked questions

Which specialties have consultation templates?

General medicine, neurology, cardiology, dermatology, paediatrics, obstetrics and gynaecology, ENT, ophthalmology, endocrinology, psychiatry and dentistry. The default texts are a starting point the doctor edits, and the clinic can set its own defaults.

Do I see what I recorded at the previous visit?

Yes. At every step of the consultation you can open the history panel and see what was recorded before for that section: texts, measurements, diagnoses, medication, treatments or allergies, with the date and the doctor. Vital signs also show as a trend chart over time.

How is the medical letter printed?

Through the browser's print dialog, so on any printer your computer or phone can use, or as a PDF. The letter uses the clinic letterhead set up in the app.

Is the built-in GDPR consent legally ready to use?

Yapp Med includes a GDPR information and consent template you can print with the letter, or you can upload your clinic's own PDF. The template is a starting point: the final wording remains the clinic's responsibility, and the clinic can review it with its legal adviser.

Do I need Yapp Book to use Yapp Med?

Yes. Yapp Med is the clinical extension for Yapp Book and needs an active Yapp Book subscription. Yapp Book appointments appear in the clinical app automatically and can open a consultation directly.
